If you would like to register with the practice, please bring your medical card with you, but do not worry if you cannot find it as you will be able to fill in a form at reception. The receptionists will be able to help you with any questions you may have. You will need to supply two forms of identification - 1: Picture ID ie. passport, driving licence, ID card.  2: Proof of address ie. Utility bill, bank statement.  Unfortunately you will not be able to register without providing any form of ID.

All newly registered patients will be asked to complete a questionnaire and may have a consultation with the practice nurse before their registration is completed. Medical treatment is available from the date of registration. You can also request a new patient check with the nursing team. If you take regular medication you will be asked to consult with a doctor as soon as possible to ensure continuity of treatment.

In order to register with the practice you must live within the practice area.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and in addition you will be asked to provide identification.
